Wantagh, Long Island
Jenifer Kramer, MD, FACP · Board-certified Internal Medicine · 24 years in practice
Adult primary care for Wantagh and the South Shore: prevention, chronic disease management, and a doctor who has the time to listen and coordinates the rest of your care.
About the doctor
Dr. Kramer is a board-certified internist and a Fellow of the American College of Physicians, a designation given to internists recognized for excellence and long-standing contribution to the practice of internal medicine.
Her practice is built on longitudinal care: getting to know each patient over many years, keeping complex medical histories straight, and making sure specialists, hospitals, and pharmacies are all working from the same page.
Dr. Kramer completed her internship and residency in internal medicine at Winthrop University Hospital and has been in practice for 24 years. She is affiliated with NYU Langone Hospital–Long Island in Mineola (formerly Winthrop-University Hospital, where she trained) and Northwell Plainview Hospital.
How the practice works
Because the practice keeps a limited number of patients, you can consult directly with Dr. Kramer and your care team by secure email, in person, through online consultations, and by phone. That direct access makes the relationship between patient and doctor substantially stronger.
Our office acts as the link between you and your other providers. It is the single “go-to” for all of your care needs. We help with:
By taking the complexity of the health care system and the frustration of managing it off your plate, we can speed up diagnosis and treatment and resolve problems more successfully.
Membership
New patients join the practice with a membership fee of $150 per month, in addition to insurance billing. The fee lets Dr. Kramer sustain the practice as insurance payments to physicians keep shrinking, while still giving patients the time and care they deserve.
For this fee, you receive:
There is no substitute for time with a physician who knows you. In many practices you are a number on a chart, in and out in 15 minutes after a long wait. Dr. Kramer takes the extra time to understand your history and challenges, give you thorough care, and answer all of your questions.
You receive Dr. Kramer’s email address, so you can reach her after hours with a health concern or question.
In most cases, we can see you for an appointment within two days.
Once a year, review your complete health history and build a plan for your goals and concerns.
Your care outside the office matters too. Dr. Kramer acts as your advocate with other providers and specialists, helping you schedule appointments and make important medical decisions.
If you are hospitalized, Dr. Kramer works with the physicians overseeing your care.
Health care is changing fast. You should have the support of a physician who knows your health thoroughly.
